ALL kids must be listed from YOUNGEST to OLDEST or your application will be deleted!!!

Required documents include: 

• Notice of Assessment 2024
• Real physical ID for ALL household members! PHOTO ID for anyone over 18. NO PICTURES OR PHOTOCOPIES ALLOWED!!!
• Proof of address if DIFFERENT than your Notice of Assessment 
• DELIVERY ONLY: Proof of disability, mental health challenge, senior status, living in a shelter 

★ Documents to be presented by appointment only. 

★ MUST register for an appointment time 2 weeks prior to the distribution date. 

★ Appointments will be online for delivery applicants and in person for pick-up applicants.

ALL applications require a secondary contact. 

→ You must include their first name and their phone number.

→ This can be a landline, additional cell phone, spouse, family member, friend, neighbour, social worker, etc.

The name, gender & birthday in section 14 "Personal Profile" are for the person applying, NOT the kids!

Delivery is only available for select individuals: 

• Anyone in your home has a disability 
• Anyone in your home has a mental health challenge 
• You are a senior who is a guardian of minors 
• You are living in a shelter

A $15°° deposit is required for all delivery applications. If payment is not made within 24 HOURS of the application, then your form will be canceled & you will need to apply again, if space is still available.
